Voluntary throughcare case management support for young people exiting the residential component of Mac River Centre (offered to all residents dependent on length of stay). Also offered as a voluntary support to referred (accepted) young people awaiting entry to residential services of Mac River Centre on an identified 'needs' basis.
Provision of service through a tailored mix of telephone, online and in person support, generally for a period of up to 12 weeks although timeframes are negotiable on a case by case basis

Referral process

All referrals for Mac River Centre Residential Rehabilitation Program are received via Youth Justice or Youth on Track NSW
